Athletics Registration - GENESIS DMS

Dover Middle School takes advantage of the convenience of registering for athletic programming using a secure online registration platform, Genesis.

Important Registration Information:

  • Winter Athletic Registration for the High School must be completed by November 10th.
  • No students are permitted to participate in practice or an event until the registration is completed.
  • Parent/Guardian must use your email address associated with Genesis to register yourchild. (Student email addresses will not work)

How do I register? 

  1. Log in to the Genesis parent portal
  2. Click forms
  3. Click the "Athletics Forms Tab"
  4. Complete the registration form

Students who intend to participate in athletics during the 2025-2026 school year must complete the online Genesis registration form.

PHYSICAL EXAMINATIONS - May be completed by:

1. Your private physician and then turned into the school nurse or
2. By the School Physician during our Sponsored “PHYSICAL DAYS” at school

  • @ DOVER MIDDLE SCHOOL - NOVEMBER 10, 2025  (3:00PM)

PRIOR TO ANY PHYSICAL YOU MUST TURN IN THE PRE-PARTICIPATION PHYSICAL FORMS TO THE NURSE. PRIOR TO ARRIVAL FOR YOUR PHYSICAL PLEASE COMPLETE PPE HISTORY FORMS AND THE PRE-PARTICIPATION PHYSICAL FORMS.

Parent/Guardian can complete the proxy waiver form to allow your child to attend the physical exam without you if you are unable to attend. This form must be signed in order for your child to receive a sports physical if you are not with them.
